A Patient-Centered Medical Home is a new way of delivering healthcare using evidence based care.

This practice of care impacts the patient by ensuring that the care provided is based on the best available evidence. This results in safer interactions, which in return can lead to reduced hospital stays or length of hospital stays. This also helps with fewer complications.

A Patient-Centered Medical Home is a new way of delivering healthcare
* promotes a partnership between you, your physician and care team.
* provides accessible, coordinated, comprehensive, and continuous quality health care.
* ensure all of your preventive, acute, and chronic care needs are met.
* emphasis on efficient and timely service delivery.
Your Physician
* will provide medical care that is right for you.
* will direct the care team (clinical staff) to coordinate your care.
The Team
* plan for your appointment by reviewing your record and making sure all required documents are in your chart.
* make sure testing is completed, results are obtained, and you are notified of your results in a timely manner.
* follow-up on referrals; making sure appointments are kept, that specialists have information about you before your appointment, and that your physician receives the specialist's recommendations
If You Are In The Hospital Your Care Team
* will make sure the physician seeing you in the hospital has your health information
* will make sure you understand how to call if you have questions about your treatments or medications after discharge
* will make sure you return for follow-up after discharge
In return, we ask that you
* are an active participant in your health care
* take charge of your health by assuming responsibility for aspects of your care
* be prepared for your appointment by bringing a list of your medications and questions
* follow the Care Plan your team has developed for you, and let your team know when you can't
* call the clinic for advice before going to the Emergency Room in a non-emergent situation
* work with the care team to learn how to self-manage your care.
Our PCMH is committed to enhancing access to care by providing
* same day appointments, and offering expanded hours.
A Patient Portal is a secure online website that gives patients convenient 24-hour access to personal health information from anywhere with an internet connection. Using a secure username and password, patients can view health information such as: recent doctor visits, medications, immunizations, allergies, lab results and request prescription refills- just to name a few. At our website you can access the portal after receiving your activation number from our staff. We encourage you to use the patient portal to ask questions of our staff or physicians.
Questions asked after office hours will be answered the next business day. If you feel you have an emergency, please follow the directions below.
PLEASE NOTE IF YOU HAVE A TRUE EMERGENCY PLEASE CALL 911. When you call our office after hours do not leave a message on the answering machine. Listen to the tape recording and call the physician on call for immediate problems.
The practice requires a 48 HOUR NOTICE FOR ALL PRESCRIPTION REFILLS.

The Patient-Centered-Medical-Home (PCMH) is an approach used by Marcellus Family Medicine to provide comprehensive primary care for children, adolescents, and adults. The PCMH is a health care setting that facilitates partnerships between patients and their physicians, and when appropriate, the patient's family. Marcellus Family Medicine seeks to improve the quality, effectiveness, and efficiency of the care we deliver, and to ensure that the activities within our practice are focused on meeting patient needs first. In our office we use the PCMH to foster a relationship of trust between the care team and the patient, and to clearly engage patients as partners in their health care.
According to insurance reports Marcellus Family Medicine has shown great effort in contacting our patients regarding preventative care and tests that go along with their preventative care on a daily basis.
Access to care 95%, Wellness visits pediatrics 89%, Child/Adolescents 95%, Breast Cancer screenings 67%, Children lead Screening 56%, Adolescent immunizations 87%. Reviewing of medications at every visit 84.5%, Lab reviews 90%, radiology tests (labs) 100%, overall patient satisfaction survery 96%. We are working diligently to keep improving and staying on top of the new recommendations regarding your healthcare. This is done through an annual survey that the results are available upon request. The results from that survey are used to help improve on things such as follow through with notifying patients of upcoming appointments, availability of appointments, access to your physician during non-office hours. Upon request Marcellus Family Medicine will share the practices clinician/practice level performance results.

The practice requires a 48 hours notice for all medication refills.
Office hours are by appointment only. We feel that access to your primary care physician is one of the most important aspects of good healthcare. In an effort to accommodate as many of our patients as possible, we offer extended office hour appointments upon request, and occasional Saturday morning appointments. Because of our extended hours, we feel the need to visit community "Urgent Care Centers" should be minimal. We encourage you to please contact our office before going to an Urgent Care Center so that we can assist you to better coordinate your medical care.

Please call if you have questions concerning your insurance. The practice provides equal access to all patients regardless of source of payment. All patients are treated the same regarding the source of insurance. Insurance information is not reviewed by the provider except as necessary for formulary or referral needs. Uninsured patients are directed to the New York State Health Exchange (https://nystateofhealth.ny.gov/), Fidelis Care (fideliscare.org). Some age appropriate services can also be covered through local government funded programs: (ex Onondaga County Cancer Services (315)-435-3653, Planned Parenthood (315)475-5540)
